• Dental Insurance:
  
  We are happy to file the necessary forms necessary to ensure that you receive the full benefits of our coverage. However, we can make no guarantee of any estimated coverage. Since the insurance policy is an agreement between you and the insurance company, we ask all patients to be responsible for all charges. Please know that we will do everything possible to make sure that you receive the full benefits of your policy.
  
  
• Payment Options:
  
  1. A 5% bookkeeping courtesy with payment in full of entire total treatment fee prior to beginning of treatment by cash or check.
  
  2. Payment in full of entire total treatment fee at each appointment with credit card (MC, VISA, Discover).
  
  3. Payment of patient responsibility (with assigned insurance benefits) at each appointment by cash, check, or credit card (MC, VISA, Discover).
  
  4. Payment for service as provided with total patient responsibility divided into equal payments and paid over number of appointments required with payments concluded at last appointment.
  
  5. Care Credit a GE Money Company, gives you convenient payment options so you can get the procedure you want, when you want it.

If for any reason your insurance company does not pay estimated benefit, it becomes your obligation payable within 90 days of service.
